· Application must be filled out, either printed in Black ink or typed. · APPLICATION MUST BE SIGNED AT THE CLERK OF COURTS OFFICE. Your signature must be witnessed in this office. · Two color pictures, 2” by 2”, taken within the last six months are required. (These must be regulation passport pictures.) · A certified copy of birth certificate is required. This must have raised seal on it. Birth certificates can be obtained at Health Department in the county where you were born. Old passports or naturalization papers are acceptable. · Drivers licenses are needed for identification. If you are a minor and do not drive, a parent must accompany you when applying and their drivers license is used as identification. · Children 13 years and younger need to have both parents present to apply for a passport. If only one parent can be present then a Statement of Consent Form for a minor under the age of 14 (form DS-3053) must be notarized and presented to the Clerk of Courts. If a child is 13 years of age or younger they must be present when applying. If a child was 13 years or younger when they received their previous passport, they need to send their old passport and certified copy of their birth certificate with the application. If you have legal custody of a child, you will have to present a certified copy of the guardianship or court order. · Personal check or money order made payable to U.S. Department of State for: ¨ $67.00 for 16 years of age or older (good for ten years) ¨ $52.00 for under the age of 16 years (good for 5 years) ¨ $60.00 for an expedited application · In addition, there is an Application Fee of $30.00 on all passports. Payment can be either cash, check or money order made payable to the Clerk of Courts. |
