Services are designed with your family's concerns and goals in mind. The children in Help Me Grow enjoy many new and exciting experiences.

Newborn Home Visiting/ Prenatal
Provided in partnership with The Hocking County Health Department
This part of Help Me Grow provides home visitation by a Registered Nurse to all new parents in Hocking County. The purpose of the free home visit is to provide a full assessment of both mom and baby, answer questions related to prenatal, post-partum and newborn care. Also to link families with services and resources.
- Service Coordination is the process of helping families and children receive all the community and Help Me Grow services they choose for their child and family. The role of the coordinator is to collaborate with the family to reach their individual needs.
- A service coordinator can provide assistance and support to families as the family builds on their strengths and resources.
How does the process work?
Based upon a family's concerns, priorities, and resources, a service coordinator helps the family find and use community services which will benefit their child and family.
Service Coordinators also provide home visiting services. These meetings are scheduled according to the family's needs and routines. A Family Support Specialist (FSS) is also available to meet the ever-changing needs of our families. The FSS provides Help Me Grow families with linkage to other parents in the community, support groups, customized information pertaining to each family, and can be the support the family utilizes to navigate the agencies and programs within Hocking County.
Individualized Family Service Plan
The Individualized Family Service Plan (IFSP) is the foundation of services that are family-centered. It is based on your child's strengths and your family's concerns and priorities.
The IFSP is a way to share concerns, information, and needs with your service coordinator and other selected individuals. The process includes sharing information the family feels is important, planning times, transportation, and people to meet with, stating what goals the family would like to achieve, and outlining who is responsible for the steps taken to complete these goals.
The MR/DD Early Intervention and Help Me Grow Partnership
The Hocking County Board of MR/DD serves children birth to age three who are experiencing one or more delays in their development. The developmental Specialist works with children though a center-based and home-based program to facilitate and stimulate development.
In addition, MR/DD provides speech, occupational, and physical therapies at the family Resource Center.
